Integrated Clinical Services (ICS) is the largest Federally Qualified Health Center (FQHC) in Multnomah County, providing quality health services for people who experience barriers to accessing care. About the position: The Community Health Nurse provides comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the clients self care using professional judgment in comprehensive assessment, screening, nursing diagnosis, care planning, counseling, intervention, case management, health information, education, referral and evaluation. What You'll Do: 1. Direct Patient Care: Comprehensive Assessments: Evaluate patient health, identify needs, and develop care plans. Intervention and Management: Provide ongoing care for chronic conditions like depression, diabetes, asthma, etc. Medication and Treatment: Administer medications and treatments as needed. Urgent Care: Respond to urgent medical needs both in-person and remotely. Health Education: Empower patients to manage their health through education. 2. Teamwork and Collaboration: Care Coordination: Work closely with the healthcare team (doctors, nurses, etc.) to ensure coordinated and effective care. Referrals: Manage referrals to specialists and other healthcare providers. Team Support: Act as a resource for other team members, providing expertise and guidance. 3. Leadership and Mentorship: Clinical Guidance: Provide mentorship and coaching to other team members. Delegation: Assign patient care tasks to team members based on their skills and scope of practice. Community Outreach: Build relationships with community partners to enhance patient care. 4. Administrative and Quality Assurance: Documentation: Maintain accurate patient records and documentation.
